Question wording affects the evidence a survey can produce. Each item should ask one understandable thing, use a recall period people can answer, and offer response choices that fit real situations.

Write one clear question

Use familiar language, specify the relevant event and timeframe, avoid assumptions, and split double-barrelled prompts. Do not reveal the answer the research team hopes to see.

Design complete answer options

Make categories distinct, include other or not applicable only where meaningful, keep rating directions consistent, and explain how many options a respondent may select.
